I think you cant have it once . Check your Inventory if you already have one .

I don't think you should be having any problems with picking it up. You can have more than one in your inventory (at least that's how it is for me on pc, I picked up a 2nd pendant in ng+ no problem). I guess I really can't say what's wrong if you're unable to pick it up.
