No, carriers of the dark sign will always come back to life when killed.
The chosen undeads journey can be stopped though, when he goes hollow. (or i guess when you lock him up in a cell without a key)
In which case Frampt will just pronounce you weren't the chosen one and find the next undead.

Lore claims we can go insane, but as we are the chosen undead, technically we can not "die"

Yes. You will die over and over and the screen will tell you so, it will say, "You Died".

Ya know, you may not be able to die per say, but there are things that could make you delete a character.
For Instance:
Mr. Tart was telling a story about one of his 1st characters.
He made it to firelink and out of curiosity, he decided to attack the Crestfallen Warrior.
Needless to say he died countless times every time he spawned at the firelink bonfire until he decided to start over lol. He tells it better, and I about fell out of my chair laughing at his story.
I myself made it all the way to Nito's bossdoor without placing the vessel on one of my first toons. I made it back to Firelink, but I was 1 more try away from starting over.
Also, I've lost bloodstains that have taken a part of my soul.
