Dark Souls was originally planned as a Demon's Souls 2, but due to copyright issues coming up with Sony owning the Demon's Souls IP, FROM couldn't legally use any direct connections. So alot of things were designed as direct connections but due to those legal issues they had to be cut, changed, or simply made incredibly cryptic.
As it was said earlier, the Tomb of the Giants is one such connection. In Dark Souls we know that a war between Lordran and the Chaos Demons ravaged the land, and the Giants were wiped out creating the Tomb of the Giants. In Demon's Souls we know that the land of the Giants was wiped out by demons. Coincidence? Another one I really like is King Jeremiah. In Dark Souls we know he is known as the "The Legendary Exile," is fond of invading, and is clad in yards and yards of sinister golden robes. In Demon's Souls we know that the Elder of Latria was banished from the land long ago, only to return wearing demonic golden robes and an extensive knowledge of black phantoms. More coincidence?
Just because they technically aren't part of the same universe, doesn't mean they weren't intended to be
