Just about anything in Blighttown, including Quelaag.
In Demon's Souls it was those big dudes with the giant clubs in the 5-2 swamp, because you can't even freaking run and dodge them.

Try doing a plunging attack on them.
I did that once. It killed me. I didn't even damage it.
