    To access the DLC, you HAVE to place the lordvessel, correct?

    If true, how does a host get weapons & armor from the DLC and be in Anor Londo with O&S undefeated?

    Yes, you have to place the Lordvessel as you need access to Duke's Archives.

    They either had them gifted, were in NG+, or were hacking.

    Just like all of the other weapons and armor in the game, DLC weapons and armor can be used on later playthroughs.
