I've been thinking of putting this on a build for a very long time, but I see no real use for it except for a Ridiculous amount of health and having a really big window or the ultimate glass cannon. How is it supposed to be used?

You swap to it when you get low on health if your build uses a ring that won't gimp the build (like relying on Havel's for fast roll) if you remove it. If you want to be really "pro" you dodge turtle behind a bow and/or spear once you get that low but I think it's playing a bit dirty.

I don't suggest this because of how evil it is, but get a bunch of poise, use a buff like DMB, two hand anything that hits really fast, like the falchion, then spam the R1 while wearing the red tearstone ring. I've seen people use it scarily effectively.

It's best to hard-swap from another ring, such as Wolf Ring. Once you think your health is below 20%, switch from a useless ring at that point to Red Tear.
It works best with Wolf Ring, because once you're under 20% you're more or less going to be one shot anyway, so your poise won't matter.

To effectively use the RTSR on a build that you expect to use it on, then:
1) Have a large amount of VIT so the HP value required is higher.
2) Make sure you're playing a build with already high damage outputs.
3) Try to play more aggressively when activated. People get scared stiff easily.
4) [Optional] You can try a "safe" aggressive strategy, such as roll attacking, poking and magic. Even if your damage is high, anyone with the guts to trade with you will mostly likely win if they are considerably unharmed.

1. Swap to it mid fight when you get low, you don't need Wolf/Leo/BDCR when you're one hit from death anyhow. This usually either scares or annoys people, menu hardswaping is a little iffy to me, because it basically gives you another ring slot.

Sanctus wont keep you alive forever but it will help mitigate, as will curse. Yes you can stack Curse/Ring but you don't have to just having the base Vit for any starting class while cursed is good enough to give you ample time to get a single hit. Thus you don't need to use up a ring slot on dusk
