No, it wasnt surge and he never got a glove out it was the proper dragon breath move.

Then it was either a visual glitch or he was somehow glitching the head on him.

It's a visual glitch. I've had times before where I summon someone who looks like they have no helm on, then they start breathing fire, and then a few minutes later the helm actually appears on their head
