Together with summoned Solaire I have entered the Smough and Ornstein fight today, killing Smough first. After the cutscene, Solaire dead and a minute or two, Ornstein used his lightning impale on me near far right corner of the room, from the entrance position. After he threw me down on the ground, I rolled away to heal myself and turned around, but Ornstein wasn't there. I kept running around the room for about 20 or 30 seconds and after that, I just got Ornstein's soul and 50000 (?) souls. Is this common/has this happenned to anyone?

This boss fight is known to have a lot of possible glitches associated with it, and in some cases, things such as red phantoms being summoned as whites, vice versa, the boss suddenly dies, which was your case, etc. It could just be extreme lag however, that's the only time things such as that have happened to me.

Ornstein was at almost full HP when this happenned and I wasn't damaging him any further (if lag means me hitting him and his HP being taken down later in this case?). Anyway thank god, I couldn't get past these two guys for ages, haha.

Ornstein tends to jump out of the window at Smough's death. He is a very suicidal fellow.

Nope. Since Ornstein completely disappeared and his body was nowhere to be found, I didn't get the ring.

I once invaded a person that was fighting Smough.......In the hallway outside the boss gate. The host had infinite health, and when I invaded there was already a red phantom there, just staring at Smough and the host. The red phantom looked at me, shrugged, and looked back. The host then ran up to us, started to do the praise the sun gesture, and was immedietely smashed my Smoughs jump attack. Another phantom invaded, and he did the same thing I did. Looked at Smough, looked at us, we both shrugged, and watched the host get attacked more. After a minute of this, the phantoms got bored and one backstabbed the other. Then he turned his attention to me, as did the host, so I ran for it. Died by one of the phantoms doing wrath of the gods at me when I ran in his direction since I couldn't kill the host. Was very weird.

I've "enjoyed" death by falling through the environment in the Catacombs (which is basically what happened to Ornstein in the OP's example).
Right next to the Darkmoon Seance Ring, no less. I was fighting the huge skeleton near there, and he knocked me back and THROUGH the statue where the Ring is. I stood up and tried to roll out of the way but he hit me again and next thing I know I'm falling through a dark skyblue void for 10+ seconds and then "YOU DIED" appears. Nice. *rolls eyes*
Falling through the terrain is the most lame way to die, because it is such shoddy coding/modeling, esp in a game that punishes you for death like Dark Souls does.
Sad thing of it is, I dropped like 9 humanity at that location. I haven't played the character since, since that kinda killed the mood.

That has just happenned to me two days ago in the Tomb of giants, haha.

Ornstein most likely fell through a wall. ( near the elevators is where ours happened) Watched it happen on a co-op once. We all ran around the room wondering what to do, since we all saw him disappear into the wall, then he just died.

He likely fell through the world and eventually died. It happens sometimes.
