If you insist on keeping the bloodshield and cloranthy ring, then I'd recommend this:
http://mmdks.com/34mb
Some notes:
- You have pyros to chip shields. IMO carry a rapier or a spear to menu swap. Note that if you use black flame + stab followup off rapier if you break their guard you're in good shape.
- Alternatively, just menu-swap the shotel too as you need it. Only a few specific builds are going to turtle on you early, and if they're weak just use pyros.
Alternative setups:
Extra HP (1800 is very solid) http://mmdks.com/34mc
Ninja Flip (between kat and menu swap options and pyros ninja flip is obnoxious) http://mmdks.com/34md
Why 76 poise:
- You can eat a zwei hit and not get stunned.
- You win poise races vs the vast majority of weapon classes with 60 or less poise.
Great chaos fireball is a good spell, however with your dex you can combo regular GFB into black flames + kat followup and so forth. Black flame is highly recommended since it does 60 poise break and you have 45 dex to spam stunlocks. With the poise I'm giving you, you can literally *trade* with zweihander guys and unless they're in full havel's, you'll stun them first.
IMO use the ninja build, with or without grass crest, and either trade w/ BF or GC, with kat, or use pokes/flip back as needed. If someone thinks they're cool 2handing a dragonslayer spear just pressure them with rapier + shield, and don't forget to punish them with BS if they get too aggressive since that HURTS from rapiers.
Otherwise, just 2h the kat and go to town...often if you get first hit you can get 3 (90 poise break) which will stagger a lot of builds and put them at risk. The best part about this is that you *can* trade with your weapon, but if you're ninja flipping you have no need to do it.
