Does Peterus kill her? Doesn't he just attempt to and patches kicks her down the pit hoping she'd die, but she doesn't and then you rescue her later and she sits in the undead church?

I believe if you do not Kill Peterus before he kills her, he then steals her humanity and her talisman, and Having no humanity left, and already undead, she cannot return to life, also she may have never rested at a bonfire, therefore she could of woken up back in thorolund.

If you clear another area before heading off to the undead church, there will be humanity and a pendant in the spot where she's supposed to be. If you kill Petrus afterwards, he drops Rhea's Ivory Talisman. I believe that there's another set of events that ends up with Petrus threatening to kill Rhea and eventually doing so too.
@Kahldris, a corpse with the maiden set(Rhea's clothes) is found in the Duke's Archives though. So maybe she got killed, revived at firelink and got kidnapped by one of Seath's channelers and hollowed out there due to lack of humanity after being killed by Petrus?

If you do not Kill Petrus, she is taken to the Archives if you buy all her miracles, so it is likely she was taken by the Channeler, where she went hollow, and it seems that if the Chosen Undead kills someone who is hollows or is in danger of becoming one, Ie all the Npc's, that they stay dead, so its kinda crazy.
