Planning to make one, but I have no idea how to properly distribute stats. At this low SL, I know I have to equip elemental weapons, because the stat scaling at this SL is not good enough. Please provide any recommendations about weapons, armor and stats.

With elementals, it is fairly easy. Get the minimum stats to wield, add appropriate endurance (30ish for kats and lower, 40 for heavier strength weapons). Pump the rest into vitality. Consider saving a few points for pyros too. That's how I'd approach it.

Also low scaling weapons are very effective, for example the Gravelord Sword.

Check this Smough's Hammer build. It doesn't have poise, because even medium armor weights to much for 30 Endurance, so substitute FAP ring for Wolf ring and add more points to End, although I don't recommend it, because you'll have to sacrifice your already low Vit.

Have you tried using Smough's Hammer with 30 end? Seems a bit low to me. But, I haven't tried it with anything less than 40 end.

I have a build like that as well.
Currently running with a +5 lgt Falchion. I've made a +14 normal as well but I'm thinking of making a Raw +5 to be able to get a better damage output with the lower stats and still be able to buff with posion/fire/lightning resin.
But right now, the +5 lightning weapon is serving me well. Doing alot of invasions at bonfire before S&O. Constant invasions. Very good spot.
EDIT*** Just looked up the raw stats for the flachion. Very very weak. Which confirms that the Raw upgrade path is really useless. But it still can be buffed so i'll do some tests later with Greater magic weapon.
