So this just happened about 30 seconds ago.

Playing Fallout 3 on very hard. I had just started a new character. Roaming the wastes for some traders to loot, raiders to kill, etc. Anyways, I had just gotten Dogmeat from the scrapyard and I was walking North. I leaned over to my computer to look at a post on the forums, and a Deathclaw takes away 50% of my health with one hit, and ends up killing Dogmeat. I pulled out my Flamer and just sprayed it down for about 30 seconds. I ended up reloading a save from 15 minutes earlier because Dogmeat died. I was legitimately scared when it happened.

In Sonic Adventure DX, I was legitimately scared of going through the nighttime areas even though there were no hostile enemies.

-Surprisingly, Skyrim, when i innocently rushed to a big chest at the end of a cave, past a giant spider on the ceiling; of course, it didn't stay there. My worst scare.

-Also, Half-Life 2, in the bugbait chapters, because everytime i turn around i'd find a (friendly) antlion right behind me, also sometimes when zombies throw **** at me.

-Tomb Raider III in a lot of places, with really loud jump scares, that was probably the game that scared me most often
*screech* *screech* SCREEEECH DUN DUN DUN

-To have your plane suddenly burst into flames and explode when hit by a battery of 20mm cannons in War Thunder, you almost want to jump out of your chair while throwing your shirt away

-Fallout New Vegas Dead Money addon, the outside area is some old italian-ish streets with infinite zombie people in hazmat suits that throw spears at you and may come back to life when they die, they're also intelligent, because they can somehow always find you, and are almost impossible to sneak by, streets also include a death cloud that constantly depletes your health, your friends include: another breed of zombie, a girl who had her vocal cords removed, and a split personality muscle mountain mutant that's fighting over the control of himself when he's not eating people, all of that while wearing a collar that explodes if you go near speakers(some of which are indestructible), radios, tries to play dumb or if one of the friends above die. later, you go into the casino that has (invincible) holograms that wear riot armor and look at you and turn orange as soon as they see you, accompanied with the usual Scare Chord, they then proceed to shoot you with lasers, that come out of their heads; even later, computer that manages them screws up and mixes their AI with the many holograms of an actress stuck in her room and crying for help, that means she also shoots you with lasers out of her head, while crying for help.

You also happen to lose all your equipment before starting the addon

hmm, I think playing RE4 for the first time really scared me, the regenerators part mostly.

I can't play games like minecraft without getting nervous. In fact, games that has a dark path and you need to go through it, makes me anxious, specially if you lose something when you die. I got a bit scared in DkS first time, at ToG and izalith.

when I was little, I used to play a ton of PS1 games and some of them gave me nightmares, sadly (or not), I don't remember them

Well I was pretty scared when I played Slender for the first time because of the soundtrack.
I was also scared in Receiver, a game in which guns work like in real life and the tension was mostly because everything could one hit you.

Have you guys played dead space 2? That game was designed to scare people. Like seriously. Just walking along durdurdurdur and you're looking around and you hear footsteps in the vents above you. All of a sudden BOOOOMM BOOM SPLASH CLASH a window breaks and you're almost getting sucked into space! You recover, your nerves are shot, you start to calm down and everything is fine. 5 minutes later A FREAKY LOOKIN MONSTER JUMPS INTO YOUR SCREEN AND SCREAMS IN YOUR FACE. You shoot the hell out of it, stomp it's face and get health.

Excluding shark-related incidents because my phobia forces them all to be frightening, I'm going with the first time I was told to hunt a Qurupeco in Monster Hunter 3 Ultimate in High Ranked missions (some of you already know where I'm going with this).

Now for those unaware, the Qurupeco is a bird-like enemy that can mimic the cries of other monsters in the area and summon them to help it out as it fights you. Usually it just summons some little mooks or maybe a medium-sized bear or lizard. Not this time. It summoned what can best be described as a T-Rex with Bane's self-controlled super steroid berserker powers (think the comics, not the Nolanverse Bane) called the Deviljho which is an endgame level monster when I was just barely into the midgame. What scared me though is that the I didn't see the thing enter the arena, as I was preoccupied with this bird I was hunting, and suddenly the music changed to the Deviljho's theme. I'm on my phone right now so I can't link, but go look up the Deviljho theme on YouTube and image not knowing why this suddenly started playing or what it meant.

Slender and Haunt are both terrifying. Pretty much every second of both of those games. You have to play them properly for em to really get you though. Must be done at night, in the dark, alone, with headphones. Amnesia does it too under those conditions. I've been trying to play Haunt for months, and I've gotten maybe 25 minutes in tops because I get scared and manufacture a reason why I want/need to do something else.

5-2 In demon's souls. My first encounter with the meat cleaver black phantom is one of the few moments i still remember from my first DeS playthrough. 5-2 in its entirety is a piece of art.

Also, I still haven't been able to finish Amnesia: the dark descent. It's so stressful and scary.

How about the entire game? I was five the first time I saw it and it gave me nightmares for years.I couldn't play through the entire game for a decade after.
And Mr X was a total nightmare, walking along, minding my own business and he busts through a wall after me.

The start of Dead Rising,I hadn't gotten over my horrible fear of zombies yet and had no clue what to expect inside the mall.Must have sat on the rooftop for ten minutes, trying to work up some courage.Glad I did because that game got me past those fears.

Any of the dozens of games that has giant spiders as an enemy, still creeped out but getting over that fear as I slice them all into tiny bits.

And to add yet another zombie experience,my first horde in Left 4 Dead.Having your speakers up loud while you sit in the dark makes you s**t a brick when all the screaming zombies come sprinting out of nowhere.
