From what I understood, the "infection" won't actually occur until the infected person dies/rests at a bonfire which respawns all the enemies throughout the entire game.
I could be wrong on that, but it would explain why people can see the sign but not the Black Phantoms.

That seems to be the case. The handful of times I've encountered BPs, they've never just appeared out of nowhere. It was always after resting at a bonfire.

There are quite a few videos and account of BP appearing out of thin air with out sitting on a bonfire. If someone on this thread wants to see it I can provide the link.
Some have also stated that upon using the dried finger BPs appeared out of nowhere
I have only seen them after I sit a a bonfire but I think that is just coincidence. I think it is probably dependent on how good the internet connection is.
