Also, GTA 4 was released in 2008. Demon's Souls was released in 2009.

Seems more coincidence than reference. The Old Monk's giant demonic heart was too finely tied into the lore and level design for me to think it was just thrown in for the sake of a reference just a year before the game's release. I just can't imagine the dev team conceptualizing and building a level from scratch that quickly for what would basically be a quick in-joke.
