Is she stronger than them? More damage more health? If she is Id say they're her followers. If she isnt she may still be there leader, but it might be another factorlike gender. Also the ***** are big fat guys. mildred looks human. Maybe they're diseased and shes looking for Yulva?

She hits harder, is more dangerous, as well she can't be poisoned by the swamp water like other NPC's. Maybe not a leader, but just a member like you mentioned. Those other butchers are both at work as well. Mildred is off hunting, one is waiting for prey to come to him, and the other is cutting up meat. Maybe they're a small community really.

If she's imune to the water and runs through it without being slowed maybe shes a natural inhabitant. Maybe shes the first pure human born there in decades. Shes got all her brethrens immunities and strength without the natural inbreeding diseases that make them look so fat.

But their skin is purplish grey and they're tube shaped. Also they are hollows. You can see the rings in their eyes. She's pale and roundish and not a hollow. They might be like her servants now. When they turned their last thoughts were to serve her so they catch food for her.

That's a possibility, or they were her comrades and merely went hollow so they are just continuing their existence. Remember, there are two hollows not far from the depths who are actually quite conscious. Both of the undead merchants, they are hollow.

Good point. So what have we decided? Laur was for her. The three were a little family of butchers. the other 2 went hollow but continue to work with her. When she attacks you shes doing it for food. Why is she different to them? Why is she down in the swamp? Why does she help you kill Quelaag?

Maybe she wants to make sure they keep their food supply coming and making sure there is less competition. as well some can take the back way into Btown which would cause the butchers to miss them, so she is making up for that. Possibly being that she is also getting fresher kills on actual humans rather than hollows like the butchers might be, she is keeping herself in a human state.

Also, anyone have any ideas about the Server in Blight town? Occult energies?

- A possible natural born denizen of the Great Swamp and cannibal Mildred has sort of a mysterious air about her that some are drawn to. While it is simple in that she is hunting for wandering souls to kill and feed upon their flesh it is also of interest that she is similar to the butchers in the depths. As well one guards Laurientus of the Great Swamp, a man who fears being eaten by some female. The Spider Witch and the Gaping dragon are possibilities, yet these hollow men have no relations to them. While they share the weapon and clothing of Mildred. It's possible she leads them, or that they are merely together in a clan killing together and feeding upon the meat of humans and possibly dogs or rats if food is scarce. Something of interest is that she is actually immune to the poisonous water of the swamp, even Queelana herself isn't immune. It is possible she has escaped the state of becoming hollow via her feeding on living souls entering the swamp of blight town. She seems to care little for modesty and instead only seeks to kill and feed rather than worry about her appearance.
Also, anyone have any ideas about the Server in Blight town? Occult energies?

If you look from Firelink you can actually see the great Swamp, and it looks exactly like lower blight town.
EDIT: As well, it leads to below Firelink, meaning Btown is probably just a closed off part of the great swamp.
